Initiation (triggers): Ectopic foci initiate rapid repetitive discharges due to enhanced automaticity or triggered activity (delayed > early afterdepolarization).

The predominant triggers are located in the pulmonary veins (PVs). The PVs contain sleeves of left atrial (LA) myocardium: These sleeves extend ~1.5–2.5 cm beyond the LA–PV junction.

These sleeves are thickest at the venoatrial junction and are longest in the superior PVs, with the left-side PVs having longer sleeves than the right PVs.
